Drivers who have been approved need to email 
or fax the following documents to Kim before 
they may attend orientation:

1.              Drivers License

2.              Truck Inspection

3.              Registration/Title

4.              Insurance

5.              DOT Physical

6.              Birth Certificate

7.              MVR Authorization

8.             A DOT drug screen must be done the day before orientation.

Drivers must bring in all of the following ORIGINALS
 the day of the orientation.

1.              Social Security Card (Federal requirement)

2.              Proper driver's license as required by your state.

3.              Current D.O.T. physical ((Long form and medical card) -  Please note you CANNOT drive a CMV if you have an impairment/loss of limb w/out a skill performance evaluation certificate)) *You are disqualified if you have monocular vision or have insulin-dependent diabetes*

4.              Current D.O.T. Truck Inspection ( State Inspection if required). There are several inspection stations near our terminal

5.              Copy of truck title (if available)

6.              Copy of truck registration* in your name. We cannot use a Bill of Sale. If your truck is not registered to you, we require a Power of Attorney signed by both parties. Call if you need this form. 
*PAPER TEMPORARY REGISTRATION NOT ACCEPTABLE.

7.              Copy of Declaration page of Commercial truck insurance showing a minimum of $500,000 liability insurance and expiration date.. not a binder.

8.             Camera must be carried at all times, digital preferred.

9.              Cell phone

10.          Emergency equipment

11.         Information for application ( mandatory: complete names, addresses, phone numbers and dates employed for the last 10 years)

**Failure to bring the above documentation will delay orientation**

Note:  If you live in CA, NH, PA or WA, you MUST bring a current Motor Vehicle Report from your license branch with you (within the last 30 days).

**Note** You need to have a CDL-A if the combined GVWR sticker rating on your truck and trailer you are transporting are over 26,000#.

balance of pay immeditaly upon turning in required paperwork.

passes 100% of the fuel surcharge to you.

provides cargo insurance and liability coverage while under load. Provides legalization for all US and Canadian loads.

offers log orientation classes and updating log classes.

has required orientation classes where you will be provided a copy of THE FEDERAL MOTOR CARRIER SAFETY REGULATIONS book.

provides National Safety Council sanctioned classes. Insurance companies in most states will give a 10% deduction in premium rates for having satisfactorily passed this course.

uses the P. C. Miller program to compute accurate "practical" mileage for all runs.

Hoosier recognizes that you are an independent owner operator responsibile for your trucking business operation. You lease your equipment on with us and provide your services to deliver RVs. Therefore, we do NOT deduct taxes but will issue a 1099 at the end of the year.

 

REQUIREMENTS ARE AS FOLLOWS:

Must have at least 2 years towing experience (not personal).

Must be at least 26 years old or be able to provide satisfactory 2 years verifiable commercial experience.

Have the proper driver's license for your state.

A CDL-A is required if your truck and the trailer that you are transporting are rated over 26,000#s GVWR. Call us if you have any questions.

Have an original Birth Certificate or a "US Passport Card".

Have an excellent driving record.

Never had a DWI, DUI, OWI, etc.

Have a current D.O.T. physical (there is a local clinic here.)

Be able to pass a drug screen.

Current D.O.T. truck inspection (there are several inspection stations near our terminal) and state inspection, if required by your state.

Valid truck registration/title.

Have the required D.O.T. emergency equipment.

Must be able to pass a road test.

Required to attend our National Council Safety Course within 30 days of leasing on OR have proof of having passed an NCSC certificate issued within the past three years by another company.

Must have a copy of your insurance policy showing a minimum of $500,000 liability unladen on your truck.

Owner operators with trucks having a GVWR over 26,001# will be responsible for their quarterly fuel taxes.

No workers comp insurance is required because Indiana law does not require independent owner operators to have it.

$100 deposit is set up for your transport plates. We will be glad to deduct $25 from your first four trips' settlement checks. THIS DEPOSIT IS REFUNDED IMMEDIATELY WHEN YOU DELEASE AND TURN IN YOUR TWO PLATES.

An escrow account will be established for every owner operator. This account will allow you to take advances on your runs and cover any applicable damages that are your responsibility. NO MONEY IS NEEDED UP FRONT. Single trucks escrow accounts are $2,000, multiple trucks (haul & tow trucks and lowboys) are $3,000. We will be glad to deduct 10% out of each disbursement check until you have established your escrow account.

  US Singles (82% of Gross*) - $1.24 to $1.41 mile.**
                                                  Plus 3 cents a mile for loads requiring a CDL A
                                                  Plus loaded tolls providing they match your logs.
                                                  Plus $ for washes if dealer required.

  Canadian Singles (82% of Gross*) - $1.33 to $1.50 mile.**
                                                  Plus $100 bonus (every 5 trips).
                                                  Plus loaded tolls providing they match your logs.
                                                  Plus $25 Border Crossing.

  * with safety/longevity Bonus increases over the first three years.
  ** normal return units from dealers pay 75% of outbound freight.

  US Haul & Tow Loads (85% of Gross) - $1.82 to $2.13 mile.**
                                                  Plus loaded tolls providing they match your logs.
                                                  Plus $ for washes if dealer required.

  Canadian H&T Loads (85% of Gross) - $2.05 to $2.21 mile.**
                                                  Plus loaded tolls providing they match your logs.
                                                  Plus $100 bonus (every 4 trips).
                                                  Plus $25 Border Crossing.

  US Lowboy Loads (85% of Gross) - $2.09 to $2.67 mile.**
                                                  Plus loaded tolls providing they match your logs.
                                                  Plus $ for washes if dealer required.

  Canadian Lowboys (85% of Gross) - $2.34 to $2.75 mile.**
                                                  Plus loaded tolls providing they match your logs.
                                                  Plus $100 bonus (every 3 trips).
                                                  Plus $25 Border Crossing.

  We pass 100% of the fuel surcharge on to YOU.

  After the 1st year without a major claim your pay is 83% of gross.
